What changed, dated on the day it changed

Every event carries a date basis. Effective is the day the thing happened or was true of (a closing, a plan year end, a policy as-of); report date is the day a document disclosed a holding with no date of its own, and those stay out of this list unless you ask for them. Schedule C relationship changes are inferences: a provider paid under $5,000 drops off the schedule.
